Tattooing, Acupuncture and Cosmetic Services

This form is to be used to register premises and/or practitioners of Tattooing, Acupuncture, Semi-Permanent Skin Colouring, Cosmetic Piercing or Electrolysis. You can register a new premises including a first person (practitioner) and/or register additional persons (practitioners) for the new premises or for existing registered premises. If your requirements are not covered, please contact us using our general contact form.

In conjunction with completing this online form, you are required to complete and upload the Tattooing, Acupuncture and Cosmetic Services application form.

• Your application will not be processed until payment has been received

Note: If an application for more than one activity is being made at the same time, only one fee is payable.
