    Note: You must be eligible to work in the UK to register for training.

    You will need to provide the following information so that we can deal with your request:

    1.  Confirmation that you have read and understood the pages specified above.  Yes / No.
    2.  Confirmation that you are eligible to work in the UK.
    3.  Block of pre-registration training you wish to attend.  Month and year.
    4.  Your date of birth.
    5.  Your contact telephone number.
    6.  Your local authority.
    7.  Any additional requirements to enable you to attend our training sessions, e.g. access requirments.
